Lichen sclerosus is a chronic scarring dermatosis of uncertain etiology affecting individuals of all races. It manifests clinically as white plaques with epidermal atrophy. Lesions occur mostly on the genitalia, perineum, or perianal skin. Extragenital lesions tend to affect the neck, shoulders, and upper torso. Lesions may also affect the skin around the eye and other areas of the face.

Lichen sclerosus may be pruritic, but cases may also be asymptomatic and represent only a cosmetic concern. There are no reported cases of malignant transformation in extragenital lichen sclerosus.

Rarely reported ocular associations with lichen sclerosus include Brown syndrome and keratoconjunctivitis sicca. In Brown syndrome (bilateral superior oblique tendon sheath syndrome), there is limited elevation of the eye when it is adducted. It has been reported in a female patient with widespread lichen sclerosus / morphea overlap where skin induration impacting eye muscle function was thought to be responsible.
