Synopsis

Balanitis xerotica obliterans (BXO) is the term used for lichen sclerosus that involves the glans penis and the foreskin causing atrophy and shrinkage of tissue. It is an inflammatory disease of unknown etiology, which progresses very slowly. The glans penis and the inner aspect of the foreskin are the areas most commonly involved.
Phimosis is commonly associated with BXO and is often the presenting complaint. Otherwise BXO is usually asymptomatic but patients may experience itching, burning with urination, painful erections, narrowing of the urinary stream and diminished force during urination.
