Synopsis

Telangiectasia macularis eruptiva perstans (TMEP) is a form of cutaneous mastocytosis that results from mast cell accumulation in the skin. These pruritic lesions persist over time and can be found anywhere on the skin surface. Usually, the patient is asymptomatic from both a cutaneous and a systemic standpoint, but, in many instances, there may be underlying systemic mastocytosis with involvement of gastrointestinal, musculoskeletal, or pulmonary systems, which has been shown to correlate with serum tryptase levels. TMEP almost exclusively affects adults, but it has been described in children. It follows a benign course, and treatment is symptomatic.

The 2016 World Health Organization (WHO) classification of mastocytosis does not mention TMEP as a separate form of cutaneous mastocytosis: it has been folded into the maculopapular cutaneous mastocytosis / urticaria pigmentosa category in this classification system.
