Synopsis

- Neonatal lupus erythematosus – Beginning in the first few months of life.
- Discoid lupus erythematosus – Predominantly skin lesions during childhood.
- Subacute lupus erythematosus – Skin lesions during childhood.
- Systemic lupus erythematosus
- Lupus panniculitis
In a large series, about 5% of patients with DLE had lesions starting in childhood. Cases have been reported in several ethnic groups, although there are less than 50 cases in the literature. A minority of patients have developed systemic lupus erythematosus (SLE), but follow-up time has usually been limited.

Differential Diagnosis & Pitfalls
- Neonatal lupus – Begins in the first months of life.
- Subacute and systemic lupus – Usually not scarring.
- Trichophyton tonsurans infection of the scalp (tinea capitis) – Can be very inflammatory and should be ruled out. A rare dermatophyte infection causes favus, which has an associated scarring alopecia.
- Perioral dermatitis and rosacea
- Sarcoidosis
- Lichen planus
- Lichen planopilaris
- Dermatomyositis
- Syphilis (see secondary syphilis)
- Porphyria (see erythropoietic protoporphyria) – Erythropoietic porphyria starts in childhood and is common.
